Adverse side effects of colon hydrotherapy can include:

- Abdominal pain
- Fullness
- swelling
- Nausea
- vomiting
- perianal pain
Some herbal preparations used in hydrotherapy have been associated with cases of aplastic anemia and liver toxicity. Rectal perforations and widespread abscesses have also been reported.
